Learn how to create a form for your survey.
⭐ Form = A form is the framework for your questionnaire which you can customize according to your own desire. |
In Step 3 "Forms" of creating your survey, you can set up your input form. Click on "Create new form". Now you have two options:
Option 1: "Select template". This option is only available if you already have forms created in the past and saved them as a template.
Option 2: "Create blank form". If you click on this, the following window will pop up.
![]() | First, you need to give your input form a name, e.g. Demographics. |
![]() | Choose an Export label, e.g. DEMO. If you export your data later on, this will be the corresponding label. |
![]() | We recommend copying the name of your input form into the description. |
Why should I copy the name of the input form into the description? When setting up the workflow, you can give the “tabs” a different name than the input form itself. For example if you add an input form that is called “Health Status” to your workflow you can name it “Visit 1”. When the input form “Health Status” also has the display label added into the description then next to the “Visit 1” will pop up a little information box to hover over. This will show you the description of your form. This is a very useful tool to know which input form you have integrated into the workflow especially if you use the same input form multiple times. |
![]() | You can choose whether this form should be published as a template for this survey or for all surveys at instance level. When creating a new form, you will have this form available as a template. |
![]() | Here you can exclude specific roles from accessing this input form. |
![]() | This feature allows you to instruct the system to save the entered data only if all required input fields have been completed to 100%. |
![]() | Finally, you can easily identify any creation errors in the preview mode. |
Below, you will continue with your Input Form Content which is created via drag and drop.
- Drag the desired input field type into the grey dotted field on the left.
- Now click on the respective input field on the left to edit it further.
- A pop-up window will open.

![]() | The Database label is filled in automatically by the system and does not require any further attention. | |
![]() | The Export label field is also filled in automatically by the system. However, it is advisable to create unique export labels. When exporting the data (e.g. as Excel or CSV), these labels are displayed in the column headings. | |
![]() | If you enter a description, this text appears in light grey below the input field. | |

![]() | You can fill in the placeholder especially if you want to show the end users an example of an entry. This example is displayed in light grey and disappears as soon as the end user starts entering something into the field. | |

![]() | As soon as you check the "required" box, this input field (or the question in the eCRF) is marked with a red star. This marking has an impact on the progress display in the general survey overview. This is discussed in more detail in the following article: Survey Overview | |
![]() | If you save an input field as "reusable", this input field will appear in the list of "Existing fields" above the "New Fields". | |

![]() | For the input field types "Single Choice", "Multiple Choice" and "Dropdown List", the first two selection options are entered by the system as examples. You can delete this text and enter you own options. | |

⭐ If you change your Display label at a later stage, the Export label will also change automatically. This can easily be circumvented. Before revising your Display label, go to the line of the Export label and enter any character. You can then change your Display label. But make sure to remove the additional character from your Export label afterwards. |
